Clarion Hospital dba Clarion Hospital EMS Station #4, 518 Penn Street, New Bethlehem, PA  16242, south side of the ambulance garage area, on or about April 14, 2015 and times prior thereto:  Three large oxygen cylinders were stored along a wall between two garage doors.  The oxygen cylinders were not properly secured to prevent the cylinders from tipping and/or being knocked over.  The chain used to secure the cylinders was arranged in a manner in that the chain did not tightly nest and/or secure the cylinders and could allow the cylinders to fall sideways along the wall.

The cover plate for the box was broken and hanging from the battery charger's flexible cord.  The receptacle box was not equipped with a cover plate.    B.)  Clarion Hospital dba Clarion Hospital EMS Station #4, 518 Penn Street, New Bethlehem, PA  16242, sleep quarter's, on or about April 14, 2015 and times prior thereto:  An electric baseboard heater contained an electrical splice that was not enclosed by a box or cover plate.  The wires and wire nuts were exposed at floor level at the base of the heater.    C.) The items did not rest squarely on the rack and by 5-3/4 inches.  B.)  Clarion Hospital dba Clarion Hospital EMS Station #4, 518 Penn Street, New Bethlehem, PA  16242, ambulance garage area near stairs, on or about April 14, 2015 and times prior thereto:  Items including, but not limited to, a Dirt Devil vacuum cleaner, a soiled linen bin and a basketball hoop, were stored within two feet of the front of a Reliance natural gas hot water heater / tank.  The manufacturer requires a minimum of 24-inches of front clearance for servicing.
