Additionally, the line-of-sight to an exit sign must clearly be visible at all times. 1) Facility: The line of sight to the exit discharge was not visible in many areas of the facility. 2) Stairway: There was no exit sign in the stairway indicating what level the exit discharge was on.

29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or were not packed or stored to prevent deformation of the facepiece and exhalation valve: Location: Finish Room Employees who were voluntarily wearing respirators were applying lacquer and sealant to the caskets and storing their respirators out in the open, exposed to hazardous chemicals, and hanging from their straps, which could affect their elasticity and, therefore, the fit.
