29 CFR 1910.145(f)(3): Tags were not used as a means to prevent accidental injury or illness to employees who were exposed to hazardous or potentially hazardous conditions, equipment or operations which were out of the ordinary, unexpected, or not readily apparent and as such tags were not used until such time as the identified hazard was eliminated or the hazardous operation was completed: a. Site - Employees were not protected from crushing and struck by injuries where synthetic web slings used for rigging were not tagged and taken out of service after sustaining excessive damage and showing signs of deformation.
