29 CFR 1904.39(a)(2): The employer did not report the in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.   On March 28, 2015, at the worksite located at Exit 71 off of Interstate 55 near Blytheville, Arkansas, an employee sustained an injury resulting in hospitalization.  The employer failed to report to OSHA the work-related in-patient hospitalization within twenty-four hours as required.
