29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:  On or about April 9, 2015:  a) Employee performing preventative maintenance to the equipment on the concrete plant was exposed to amputation hazards when the employer failed to properly train the employee on lock out tag out procedures.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  On or about April 9, 2015:  a) An employee was exposed to chemical burns to skin and eyes while using ZEP Mudslide RTU a corrosive concrete cleaning chemical. Employer failed to properly install and maintain an emergency eye wash station for employees using chemical to clean vehicles

29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:  On or about April 7, 2015:  a) Employees were exposed to chemical irritants when the employer failed to properly train employees on the in place hazardous communication program, to allow the employees to properly protect themselves from the hazards associated with corrosive chemicals to include Hammer Clean XT.
