A. Schaub Excavating, Inc. worksite located at 222 S. Blanchard St., Wapakoneta, Ohio: On or about April 13, the employer failed to ensure a safe means of egress such as a ladder was provided to employees installing sanitary lines in a trench approximately 5.50 feet deep.
