29 CFR 1910.132(f)(1): The employer did not provide training to each employee who is required by this section to use personal protective equipment:    a) The employer failed to provide employees performing tree trimming and chipping operations training on the use of, inspection of and care of personal protective equipment, such as hard hats.    b) The employer failed to provide employees performing tree trimming and chipping operations training on the use of, inspection of and care of personal protective equipment, such as eye protection.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.133(a)(1): Protective eye equipment was not required where there was a reasonable probability of injury that could be prevented by such equipment:    a) The employer failed to require the use of proper eye protection, such as safety glasses, for employees that were exposed to flying debris during the use of the Vermeer SC252 stump grinder and the Mobark Twister 12 Brush wood chipper.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects:    a) The employer failed to require the use of head protection, such as hard hats, for employees that were exposed to falling objects and debris during tree trimming operations and from being struck by branches that were being chipped when operating the Mobark Twister 12 Brush wood chipper.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
