29 CFR 1904.39(a)(2): The employer did not report the in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. On February 02, 2015, at the worksite located at the intersection of Interstate 30 and Roosevelt in Little Rock, Arkansas, an employee sustained an injury resulting in hospitalization. The employer failed to report to OSHA the work-related in-patient hospitalization within twenty-four hours as required.
