29 CFR 1926.501(b)(5): Each employee on the face of formwork or reinforcing steel was not protected from falling 6 feet (1.8 m) or more to lower levels by personal fall arrest systems, safety net systems, or positioning device systems:    On or about 3/18/15    Location: 160 Main Street, Tuckahoe, NY 10707    a) Employees working on Symons Steel Ply concrete formwork at heights up to 12 feet were not protected from falls. There were no approved anchor points installed on the formwork for employees to attach their positioning device system or fall arrest system to.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On or about 3/18/2015    Location 160 Main Street Tuckahoe, NY 10701    a) Employees used the base section of a Werner 32 foot extension ladder to access the top of a foundation wall and bracket scaffold at a height of approximately 12 feet. The ladder extended above the landing only 1.5 feet.
