29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a. On or about 04/15/2015, in the fabrication shop, employees were exposed to an amputation hazard, in that, while cutting plastic parts the guard on the Sears 12 inch 2 speed band saw was not adjusted to the 1/4 inch plastic parts leaving 2.5 inches of the blade exposed, on or about 12/09/2014.  b. On or about 04/15/2015, in the fabrication shop, employees were exposed to amputation and struck by hazards, in that, the Dayton Shopmaster drill press did not have a cup guard.

On or about 04/15/2015, employees were exposed to an amputation hazard, in that, the Sears Craftsman 12 inch band saw would resume operation automatically when the power was restored.

On or about 04/15/2015, in the production area, employees were exposed to a shock hazard, in that, the circuit breaker panels were not entirely labeled.
