29 CFR 1910.176(b): Secure storage. Storage of material shall not create a hazard. Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse.    Throughout Facility - On or about April 15, 2015, employees were required to work near and around racks containing car parts and other items.  The employer had not secured the racks and products from sliding or collapsing, thereby exposing employees to struck by hazards.

29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).    Throughout facility - On or about April 15, 2015, employees were required to operate powered industrial trucks on a daily basis.  The employer had not ensured that operators were trained and evaluated to safely operate powered industrial trucks, thereby exposing all employees in the plant to struck by hazards.

29 CFR 1910.178(q)(7):   Industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found shall be immediately reported and corrected.  Throughout facility - On or about April 15, 2015, employees were required to operate powered industrial trucks on a daily basis.  The employer did not perform an examination of the power industrial truck to check for defects after each shift, thereby exposing employees to serious struck by hazards.

Flexible cords were used as a substitute for fixed wiring of a structure on the packaging line, thereby exposing employees to electrical fire hazards and smoke inhalation.

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment (PPE):    On or about April 15, 2015, employees were exposed to eye and skin hazards during dismantling of cars, exposing them to gasoline, antifreeze, and various oils.  The employer had not perform a workplace hazard assessment, select and have each affected employee use PPE, communicate selection decisions to each affected employee, train each affected employee and verify that the required hazard assessment was performed.
