29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system.  Employees performing roofing activities on a residential framed structure were not using fall protection and exposed to an unprotected fall hazard at a height of over 6 feet to the ground below.

On or about April 15, 2015, the employer did not ensure employees engaged in roofing activities were trained to recognize and avoid fall hazards and the procedures to be followed in order to minimize or avoid those hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about April 15, 2015, employees installing composition roof shingles on a residential roof were using a ladder that was not extended at least 3 feet above the upper landing and exposed the employees to an unprotected fall of over 10 feet to the ground below.
