29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Midwest Metal Fabricators Ltd.; On or about December 26, 2014 the employer failed to ensure that employees operating the Niagara slip roll forming machine were protected from in going nip points. Employees were exposed to caught in hazards including, but not limited to amputations, in that in going rotating rollers were un-guarded. An employee suffered an avulsion injury after their left index finger was pulled into the in-feeding rollers of the Niagara slip roll forming machine as sheet metal was being fed into the machine.
