On or prior to 4/14/15, the employer had failed to provide fit testing to an employee that had been assigned a 3M tight-fitting, half face respirator. The respirator was provided by the site manager as a protection against airborne particulates.

29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or were not packed or stored to prevent deformation of the facepiece and exhalation valve.  On or about 4/14/15, in the Stone Room, the CSHO observed a 3M Half Face, tight-fitting respirator with a 2091 P100 particulate filter belonging to an employee. This respirator was hanging on the wall and was within approximately 20 feet of the saw where stone slabs are cut and processed. This respirator was not maintained in a protective covering and dust was caked on the outside and inside of this respirator.

On or about 4/14/15, in the Stone Shop, the CSHO observed an extension cord with a damaged outer and inner wire covering was utilized to power a Bosch Model 1710 Grinder. A portion of the metal wiring was visible.
