No ladder was provided to access the work platforms of 9 feet, 9 inches. Employees, either climbed on the cross braces or stepped on the lowest scaffold frames that were 3 feet 6 inches high.   NO ABATEMENT CERTIFICATION REQUIRED

The mobile scaffold with 9 feet 9 inches working platforms had 2 wheels locked diagonally, the other 2 wheels were not locked.   NO ABATEMENT CERTIFICATION REQUIRED

Employee, on a mobile scaffold of 9 feet 9 inches work platforms with 2 wheels locked diagonally; moved the scaffold himself while the employee was on the platforms, that resulted in the employee fell to the ground. Employee, on a mobile scaffold of 9 feet 9 inches working platforms; was not trained to recognize the hazards, that resulted in the employee moved the scaffold with 2 wheels locked while he was on the platforms, that resulted in the employee fell to the ground.   NO ABATEMENT CERTIFICATION REQUIRED
