29 CFR 1926.850(a): Prior to permitting employees to start demolition operations, an engineering survey shall be made, by a competent person, of the structure to determine the condition of the framing, floors, and walls, and possibility of unplanned collapse of any portion of the structure. Any adjacent structure where employees may be exposed shall also be similarly checked. The employer did not have in writing evidence that such a survey has been performed: On or about April 16, 2015, the employer had not compiled and made available for employees, an engineering survey prior to the commencement and during the entire demolition of the structure located at 315 East State Street, Salem, OH.

29 CFR 1926.1101(f)(1)(i): The employer who has a workplace or work operation where exposure monitoring is required under this section did not perform monitoring to determine accurately the airborne concentrations of asbestos to which employees may be exposed: On or about April 16, 2015, the employer had not conducted the monitoring as required while demolishing the building on 315 E. State Street, Salem, OH after being provided a survey conducted by the city of Salem, prior to the building becoming structurally unstable, that asbestos containing materials were present within the structure.
