29 CFR 1926.756(a)(1): During final placement of solid web structural members, the members were not secured with bolts per connection of the same size and strength as shown in the erection drawings: (a) On or about April 15, 2015, for the employee that fell approximately 30 feet to a lower level while performing connector work activities on steel beam no. 5060G1 where the correct connector bolts, available on site, were not installed at 414 East Clark Street, Vermillion, South Dakota.

29 CFR 1926.761(c)(2)(ii): The employer did not ensure that each connector had been provided training in the establishment, access, proper connecting techniques and work practices required by section 1926.756(c) and section 1926.760(b): (a) On or about April 16, 2015, and at times prior to that date, for the employees exposed to fall and/or struck-by hazards while performing connector work activities during solid web steel beam placement where connector bolts were installed hand-tight and the beam released from the hoisting line at 414 East Clark Street, Vermillion, South Dakota.
