29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about April 15, 2015 and at times prior thereto, employees working in and around an area where an auger with unguarded blades was in operation were exposed to amputation and caught in hazards.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:   On or about April 15, 2015 and at times prior thereto, employees were exposed to electrical shock and burns  due to 110 and 220 volt electrical control panels blocked by work station tables and saws set up. The electrical panels are not provided with sufficient space for maintenance and emergency shut down when necessary.   Instance 1: The Panel boxes mounted on the outer wall near the main front office adjacent to pallet assembly tables on the SW side of facility.   Instance 2: The panel boxes and emergency control switch located near the Ironwood cross cut saw and feed table located at far end NE side of main bldg.

29 CFR 1910.305(a)(2)(i): Temporary electrical power and lighting installation(s) 600 volts, nominal, or less were used in a prohibited manner.   On or about April 15, 2015 and at times prior thereto, the flexible power cord set used to power the auger machine was damaged and covered with wood chips, exposing the employee to fire and electrical shock hazards.
