29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Detroit Dam- (a) The Dake horizontal band saw's unused portion of the saw blade was unguarded exposing operators to serious injuries. (b) The spillway gates rotating wheel mechanisms were not guarded to prevent employees from the ingoing nip points, exposing them to serious hazards.

29 CFR 1910.215(a)(1): Abrasive wheel(s) used on grinding machinery were not used with safety guard(s) Detroit Dam- (b) The manufactures guard on the Milwaukie 14" chop saw was missing, exposing employees to serious injuries from the rotating wheel. (c) The 90 degree angle grinder's abrasive wheel was not guarded to prevent employees from flying debris, exposing them to serious hazards.
