29 CFR 1910.23(a)(9): Every floor hole into which persons cannot accidentally walk (on account of fixed machinery, equipment, or walls) were not protected by a cover that leaves no openings more than one inch wide.   Foster Dam-   (a) Floor holes in and around the stair cases entering the short term and long term walkways were not covered.   (b) Floor holes on the Crowder operator's platform were not covered.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Foster Dam-  (a) There are multiple nip points on and around power driven equipment of the Crowder's, including the travel system and the brail screen gate, and along the rail when the Crowder is traveling.  (b) The rotating parts of the hoisting machinery operating the spillway gates were unguarded and exposed employees to rotating parts.

29 CFR 1910.219(c)(2)(i): All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting.  Foster Dam-  (a) The false weir pump is not guarded to prevent exposure to rotating parts of the unit.  (b) The deck wash motor's shaft was unguarded exposing employees to rotating parts.

29 CFR 1910.212(a)(5): Fan blade guards, where the periphery of the blades was less than seven feet above the floor or working level, had openings larger than one half inch:    Foster Dam- (a) The Valve room exhaust fan blades were unguarded and exposed employee to serious injuries from rotating blades.   Foster Dam, Adult Fish Collection Facility- (b) The false weir pump is not guarded to prevent exposure to rotating fan blades of the unit.   The Army Corps of Engineers was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1910. 212(a)(5), which was contained in OSHA Inspection Number 96892, Citation Number 1, Item Number 6, and was affirmed as a final order on 09 /16/2011 with respect to a work place located at, Umatilla, OR.  Note: Abatement certification and supporting documentation are required for these items.

29 CFR 1910.144(a)(3): Yellow was not used to designate caution and marking physical hazards such as : Striking against, stumbling, falling, tripping, and "caught in between".  (a) The protruding air lines along the long term port sort pools presented a trip hazard and were not clearly marked.
