29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening: Green Peter Dam- (a) The crane access was not guarded to prevent a fall of 17', exposing employees to serious injuries. (b) The #1 Spillway Gate access platform was not guarded with a swinging gate or offset, exposing employees to serious injuries.

29 CFR 1910.23(a)(8)(ii): Every floor hole into which persons can accidentally walk was not protected by either a a removable standard railing or an attendant. Green Peter Dam- (a) The standard railing installed was not equipped a midrail, exposing employees to falls of 27' 7" through the 30 inch floor openings.
