29 CFR 1926.405(a)(2)(ii)(I): Protection was not provided to avoid damage to flexible cords and cables used for temporary wiring which passed through doorways or other pinch points. On or about 16 April 2015, and at times prior thereto, roofers were using an electrical extension cord for hand-held tools on a roof in which the cord ran through presence-sensing metal and glass sliding doors of a hotel entrance, exposing the workers to an electrical hazard of approximately 120 volts.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about 16 April 2015, and at times prior thereto, roofers were performing roofing work to install weatherproofing and flashing on several flat roof levels without the use of fall protection, exposing the roofers to fall hazards of approximately 20.5 feet, 13.5 feet, and 10 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about 16 April 2015, and at times prior thereto, roofers utilized a 32 foot aluminum extension ladder to access an upper landing of a roof that was not extended at least 3 feet above the landing, exposing the roofers to a fall hazard of approximately 13 feet 5 inches to the ground below.
