29 CFR 1903.19(c)(1): 29 CFR 1903.19(c)(1):   The employer failed to certify to OSHA (the Agency) that the following item(s) were abated within 10 calendar days after the abatement date:  a)  Initiation and maintenance of a safety and health program, b)  Ladders are installed and secured at least three feet above the access/egress level, c)  Ladders are inspected by a competent person prior to usage for visible defects, d)  Ladders are used only for the purpose for which they were designed, e)  Employees are protected from falling from the roof while performing roofing activities and f)   Employees are trained on how to recognize and minimize their exposure to fall hazards.

(a)  Employees were exposed to fall hazards while using an extension ladder for purposes not intended by the manufacturer. 29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    (a)  Employees conducting residential roofing activities on a residential roof with a 4:12 and 8:12 pitch were not protected from falling to the lower level.  This exposed employees to fall hazards of approximately 11 feet and 15 feet.     The Alliance Roofing Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 313527186, citation number 1, item number 1 and was affirmed as a final order on May 25, 2010, with respect to a workplace located at 317 W. Roberta Ave., Waukesha, WI 53186.
