29 CFR 1926.403(b)(2): Installation and use. Listed, labeled, or certified equipment shall be installed and used in accordance with instructions included in the listing, labeling, or certification.    a. Next to the "desk" a relocatable power tap was being used on a construction site, in violation of the UL listing and labeling, thereby exposing employees to an electrical hazard.    b. Next to the "desk" a relocatable power tap was daisy chained with extension cords, the power tap was plugged into an extension cord and a second extension cord was run from the power tap to where contractors were using it for portable tools, thereby exposing employees to an electrical hazard.

29 CFR 1926.405(b)(1): Conductors entering boxes, cabinets, or fittings. Conductors entering boxes, cabinets, or fittings shall be protected from abrasion, and openings through which conductors enter shall be effectively closed. Unused openings in cabinets, boxes, and fittings shall also be effectively closed.    a. Temporary electrical panel box L, had four unused openings on the top of the box, thereby exposing employees to contact with live electrical current.    b. At the top of the temporary electrical panel box L, the main electrical going into the panel and 3 romex cords leaving the top of the panel box were not protected with a fitting or bushing from abrasion, thereby exposing employees to contact with live electrical current.    c. The receptacle box mounted near panel box L had an unused opening on the right side of the box, thereby exposing employees to contact with live electrical current.

29 CFR 1926.405(d): Switchboards and panelboards. Switchboards that have any exposed live parts shall be located in permanently dry locations and accessible only to qualified persons. Panelboards shall be mounted in cabinets, cutout boxes, or enclosures designed for the purpose and shall be dead front. However, panelboards other than the dead front externally-operable type are permitted where accessible only to qualified persons. Exposed blades of knife switches shall be dead when open.    a. The switchboard inside the electrical cabinet H was not dead front, thereby exposing employees to live exposed parts.
