29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) On or about 4/14/15 at the Fairway Pointe Senior Village construction project located at 2123 Southwestern Drive, Lakewood, New York; employees were roofing a new building with a roof pitch of 5 in 12 and an eave height of 27'-8". An employee installing shingles was not protected from falling.  NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:    a) On or about 4/14/15 at the Fairway Pointe Senior Village construction project located at 2123 Southwestern Drive, Lakewood, New York; employees roofing a new building were using ropes with rope grabs attached to their harnesses. The employees did not have their rope grabs adjusted or rigged to prevent free falls less than 6 feet.     NO ABATEMENT CERTIFICATION REQUIRED
