29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  Maintenance Shop - The work rests on the Ryobi 6 inch bench grinder were inadequate.  The work rest on the left grinding wheel was approximately 1/2 inch from the periphery of the wheel.  The right grinding wheel was not provided with a work rest.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  Maintenance Shop - The both tongue guards on the Ryobi 6 inch bench grinder were not adjusted to within 1/4 inch of the periphery of the grinding wheels.  The tongue guard for the left grinding wheel was adjusted to approximately 7/8 inches from the periphery of the wheel.  The tongue guard for the right grinding wheel was adjusted to approximately 1/2 inch from the periphery of the wheel.

The receptacle was not taken out of service.    b)  Wall of Maintenance Shop, Adjacent to Entrance, Outdoors - The sheathing was damaged on the extension cord which was plugged into the receptacle outlet.
