29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:  100 Oak St., Clinton, MA: On or about April 16, 2015, several employees were roofing this residence more than 20 feet above the ground below without fall protection.   Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface:  100 Oak St., Clinton, MA: On or about April 16, 2015, several employees were accessing their roof top work location  at this residence using an aluminum extension ladder that did not extend any distance past the upper landing surface.
