29 CFR 1910.1000(e): Feasible administrative or engineering controls were not determined and implemented to achieve compliance with the limits prescribed in 29 CFR 1910.1000(a) through (d): a) Florida Laboratories, Investment Room - The bench top ventilation hood was inadequate to control dust emissions. The capture velocity was 20 feet per minute at 6" from the face. In addition, there was no ventilation provided over the blue tote that contained the Vitallium 2000 powder. An employee was exposed to respirable dust containing 64% crystalline silica (quartz) at a TWA of 0.367 mg/m3, which was 2.41 times above the permissible exposure level (PEL) of 0.152 mg/m3, and was taken for 460 minutes, on or about May 28, 2015.
