29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee. (a) Location: 315 Turnpike Street, North Andover MA, 01845 On or about 4/17/2015, employees were not protected from falling when using a PFAS system which did not have an anchor point of supporting at least 5000 pounds per employee.
