29 CFR 1910.335(a)(2)(ii): Protective shields, protective barriers, or insulating materials shall be used to protect each employee from shock, burns, or other electrically related injuries while that employee is working near exposed energized parts which might be accidentally contacted or where dangerous electric heating or arcing might occur. When normally enclosed live parts are exposed for maintenance or repair, they shall be guarded to protect unqualified persons from contact with the live parts.  a.  At the workplace located at 845 E. Markison Ave., Columbus, Ohio, Schlatter #3 resistance welding machine had normally enclosed live electrical parts that were unguarded while awaiting permanent repair, thereby exposing employees to an electrocution hazard.  On or about April 17, 2105, an employee came into contact with live electrical parts on the Schlatter #3 resistance welding machine, resulting in a fatal electrocution.

29 CFR 1910.335(b)(2): Barricades. Barricades shall be used in conjunction with safety signs where it is necessary to prevent or limit employee access to work areas exposing employees to uninsulated energized conductors or circuit parts. Conductive barricades may not be used where they might cause an electrical contact hazard.  a.  At the workplace located at 845 E. Markison Ave., Columbus, Ohio, the employer did not utilize alerting techniques such as barricades with safety signs to warn and protect employees from uninsulated energized electrical parts on Schlatter #3 resistance welding machine while awaiting permanent repair, thereby exposing employees to an electrocution hazard.

29 CFR 1904.29(b)(1): What do I need to do to complete the OSHA 300 Log? You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year.  Located at ITC Manufacturing in Columbus, Ohio:  a.  On the 2015 OSHA 300 Log the employer did not provide all information required in Column F such as the actual injury.       1.  Case 1, January 23, 2015, column F stated, "EE's fingers were caught between rollers", which is insufficient to determine what the actual injury was.        2.  Case 2, February 8, 2015, column F stated, "EE fed wire through feed roller by hand instead of using the proper tool. Rolls grabbed glove and pulled finger and thumb under the wheels", which is insufficient to determine what the actual injury was.       3.  Case 3, April 13, 2015, column F stated, "EE was installing a guide rail for a welder for the roll form.  Knee was hurting badly.  He limped and could hardly walk.  He left early", which is insufficient to determine what the actual injury was.

29 CFR 1904.31(a): Basic requirement. You must record on the OSHA 300 Log the recordable injuries and illnesses of all employees on your payroll, whether they are labor, executive, hourly, salary, part-time, seasonal, or migrant workers. You also must record the recordable injuries and illnesses that occur to employees who are not on your payroll if you supervise these employees on a day-to-day basis. If your business is organized as a sole proprietorship or partnership, the owner or partners are not considered employees for recordkeeping purposes.   a.  Located at ITC Manufacturing in Columbus, Ohio:  On the 2015 OSHA 300 Log the employer did not record recordable injuries of temporary workers supervised by the company on a day-to-day basis.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  a.  The employer did not report to OSHA an in-patient hospitalization.  A temporary employee was injured on or about April 1, 2015 that required in-patient hospitalization.
