29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: a) Employees performing roofing activities were exposed to fall hazards and the employer did not ensure all of the employees used fall protection. b) Employees performing roofing activities were exposed to fall hazards and the employer did not ensure employees adjusted their rope lanyards to prevent the employees from falling off the edge of the roof. c) Employees performing roofing activities were exposed to fall hazards and the employer did not ensure employees wore their fall protection harness tight enough and ensure the rope lanyards were positioned to prevent the employees from falling off the edge of the roof and suffering injuries related to the incorrect use of the equipment.
