29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s)  Employees performing roofing activities were exposed to fall hazards and an inspection of the job site was not made by a competent person to ensure employees wore their fall protection harness in an appropriate manner and ensure the rope lanyards were positioned in a manner to prevent the employees from falling off the edge of the roof.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:  a) Employees performing roofing activities were exposed to fall hazards and the employer did not ensure all of the employees used fall protection.  b) Employees performing roofing activities were exposed to fall hazards and the employer did not ensure employees adjusted their rope lanyards to prevent the employees from falling off the edge of the roof.  c) Employees performing roofing activities were exposed to fall hazards and the employer did not ensure employees wore their fall protection harness tight enough and ensure the rope lanyards were positioned to prevent the employees from falling off the edge of the roof and suffering injuries related to the incorrect use of the equipment.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  Employees performing roofing activities were exposed to fall hazards and the employer did not provide training to each employee to ensure they recognized the hazards of falling, the safety requirements associated with the use of fall protection systems, and the proper work procedures to follow in order to minimize or eliminate the fall hazards.
