29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded by either a standard railing with standard toe board on all exposed sides, or a floor hole cover of standard strength and construction. On or about April 16, 2015 and at times prior there-to, employees were exposed to a tripping hazard when working on-top of a metal grate covering with holes more than 1 inch in width.
