29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: a. Paralleling Switchgear, 145 King of Prussia Road, Radnor, PA 19087 - The employer did not verify the Paralelling Switchgear was de-energized. An arc-flash explosion occurred in the area where an employee who was assisting in the removal of switchgear that was energized at 4160 VAC. Noted On or about April 13, 2015
