29 CFR 1926.502(b)(3): Guardrail systems were not capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied within 2 inches (5.1 cm) of the top edge, in any outward or downward direction, at any point along the top edge: a) At the jobsite, on the roof, on or about 4/14/15: Workers were performing roofing work on an irregularly shaped roof where a cable guardrail system was installed, and where that cable system was incorrectly installed and not able to assure the 200 pound minimum force requirement. Abatement certification must be submitted for this item.

29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used: a) At the jobsite, on the roof, on or about 4/14/15: Workers were performing roofing work on an irregularly shaped roof where a cable guardrail system was installed, and where that cable system was incorrectly installed and not able to assure the 200 pound minimum force requirement nor were workers trained to recognize the faulty installation.
