29 CFR 1910.23(a)(4): Every skylight floor opening and hole was not guarded by a standard skylight screen or a fixed standard railing on all exposed sides. a) Roof of the White Mills Building - Employees stood on a 12 foot high low slope roof to repair and replace a broken skylight without the benefit of fall protection such as railings in place or the use of a personal fall arrest system thereby exposing the employee to a fall hazard, reported on or about 4-15-15. Abatement certification required.
