29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about April 21, 2015, at Val-Fab, Inc. located at 218 Jackson Street, Neenah, Wisconsin, employees assigned to weld on stainless steel vessels were exposed to ingoing nip points from unguarded trunnions located on two different tank rolling devices.

29 CFR 1910.1026(d)(2)(v): If periodic monitoring indicates that employee exposures are below the action level, and the result is confirmed by the result of another monitoring taken at least seven days later, the employer may discontinue the monitoring for those employees whose exposures are represented by such monitoring: a) On or about April 21, 2015, at Val-Fab, Inc. located at 218 Jackson Street, Neenah, Wisconsin, records review by CSHO Fogleson indicated an employee exposure above the OSHA PEL for Hexavalent Chromium in a report from General Casualty dated March 6, 2008. Employer did not conduct periodic monitoring to determine whether or not Hexavalent Chromium exposures were below the action level indicating the employer may discontinue the monitoring.
