The procedures did not include specific information regarding the scope, purpose, rules and techniques to be utilized for the control of hazardous energy; a specific statement of the intended use of the procedures; specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; specific procedural steps for testing machines and equipment to verify the de-energization; specific requirements for notification of affected employees; specific procedures for re-energization of the machines; specific requirements for inspections of work area prior to reenergizing equipment; and specific requirements for notification of employees prior to reenergizing equipment.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    a) Atco Tool & Manufacturing Co. / 1421 Baldwin St, Meadville PA: On or about April 17, 2015 in the grind area, the Delta Rockwell Sander Grinder was plugged into an outlet with a knockout style receptacle box that was not mounted/secured to the wall. The knockout style receptacle box was being used as permanent extension cord.      b) Atco Tool & Manufacturing Co. / 1421 Baldwin St, Meadville PA: On or about April 17, 2015 in the EDM between the Elite Pulistron P00CP and the Elite Pulsitron TRM-21, a Futaba Pulscale was plugged into an outlet with knockout style receptacle box that was not mounted/secured to the wall. The knockout style receptacle box was being used as permanent extension cord.      c) Atco Tool & Manufacturing Co. / 1421 Baldwin St, Meadville PA: On or about April 17, 2015 in the lathe area, a position readout for the Mitutoyo UDR 220 was plugged into an outlet with knockout style receptacle box that was not mounted/secured. The knockout style receptacle box is designed to be secured to keep it in place.      d) Atco Tool & Manufacturing Co. / 1421 Baldwin St, Meadville PA: On or about April 17, 2015 by the exit door in the saw room, a knockout style receptacle box was setting on a covered trash can and was plugged into an outlet on the wall. The knockout style receptacle box was being used as an extension cord.
