OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: a) Actco Tool & Manufacturing Co. / 1421 Baldwin St, Meadville PA: On or about April 17, 2015 and times prior thereto, employees were exposed to fire and fall hazards in the heat treat furnace room which did not have emergency lighting available to mark the exit route. The room was not configured in a way that windows could provide supplemental illumination during an emergency or power outage.

The knockout style receptacle box was being used as permanent extension cord. b) Atco Tool & Manufacturing Co. / 1421 Baldwin St, Meadville PA: On or about April 17, 2015 in the EDM between the Elite Pulistron P00CP and the Elite Pulsitron TRM-21, a Futaba Pulscale was plugged into an outlet with knockout style receptacle box that was not mounted/secured to the wall. The knockout style receptacle box was being used as permanent extension cord. c) Atco Tool & Manufacturing Co. / 1421 Baldwin St, Meadville PA: On or about April 17, 2015 in the lathe area, a position readout for the Mitutoyo UDR 220 was plugged into an outlet with knockout style receptacle box that was not mounted/secured. The knockout style receptacle box is designed to be secured to keep it in place. d) Atco Tool & Manufacturing Co. / 1421 Baldwin St, Meadville PA: On or about April 17, 2015 by the exit door in the saw room, a knockout style receptacle box was setting on a covered trash can and was plugged into an outlet on the wall. The knockout style receptacle box was being used as an extension cord.
