29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  At a residential home, employees performing roofing work used an electric circular saw to cut roofing tiles without eye protection and were exposed to eye injuries and struck-by hazards.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  At a residential home, employees performing roofing work were not provided fall protection and exposed to an unprotected fall hazard at a height of over 6 feet to the ground below.
