29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or 1926.652(c): Along a trafficked road, on or about April 17, 2015, employees were working inside an excavation installing a 6 inch fire line approximately 5 feet 6 inches deep by 7 feet wide by 14 feet long without any type of adequate cave-in protection in B type class soil.
