29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  a)  Ed Conolly General Contractor - 136 N. Main St., Sellersville, PA - An employee was exposed to fall hazards of approximately 13 feet  while working from a scaffold ladder platform  removing building material from an existing roof eave, on or about April 15, 2015.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.  Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photograph or video evidence of abatement or other written records.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a)  Ed Conolly General Contractor - 136 N. Main St., Sellersville, PA - On or about April 15, 2015, employees were working on a walking working surface with an unprotected edge approximately 10 feet above grade without a fall protection system in place, exposing the employees to fall hazards .    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a)  Ed Conolly General Contractor - 136 N. Main St., Sellersville, PA - On or about April 15, 2015,  employees were using an extension ladder to access a landing and the extension ladder did not extend 3 feet above the upper landing surface as required, exposing employees to fall hazards.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
