29 CFR 1928.57(a)(7)(i): The employer did not protect employees from coming into contact with hazards created by moving machinery parts through the installation and use of a guard or shield or guarding by location:  a) Jobsite: Mushroom House, 2nd level - An employee was exposed to amputation hazards when operating the "Puller" machine as the rope winding around the mechanical roller was not guarded.  Observed on or about 4/10/15.
