(a) Noa Brands America, Inc., 1460 Overlook Dr., Lafayette, CO 80026: On and before April 20, 2015, the employer did not ensure employees manufacturing mannequins had been provided effective information and training on the hazardous chemicals used in the workplace.  This condition exposed employees to chemical hazards such as but not limited to sensitization from contact with isocyanates.   Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
