29 CFR 1910.106(e)(6)(ii):  Grounding. Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100 °F (37.8 °C), shall not be dispensed into containers unless the nozzle and container are electrically interconnected. Where the metallic floorplate on which the container stands while filling is electrically connected to the fill stem or where the fill stem is bonded to the container during filling operations by means of a bond wire, the provisions of this section shall be deemed to have been complied with.  (A)  On or about 17 April an employee dispensed a flammable liquid with a flashpoint below 100 degrees  F into a container using a nozzle that was not electrically interconnect to the container.

29 CFR 1910.106(f)(6): Sources of ignition. Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100 °F (37.8 °C), shall not be handled, drawn, or dispensed where flammable vapors may reach a source of ignition. Smoking shall be prohibited except in designated localities. "No Smoking" signs shall be conspicuously posted where hazard from flammable liquid vapors is normally present.    (A) On  or  about 17 April an employee handled a flammable liquid (Acetal) with a flash point below 100 degrees F in a manner that allowed flammable liquids to reach a source of ignition in that the equipment used to transfer the liquid was improperly grounded.  The Lutz MD-2 fluid transfer pump was not configured as specified by the manufacture to ensure proper grounding or the  prevention of vapor release.
