29 CFR 1926.50(b): Provisions were not made prior to commencement of the project for prompt medical attention in case of serious injury: (a) Jobsite - On or about 04/16/2015, the employer did not ensure that adequate provisions for prompt medical attention were established prior to the commencement of the project in the event that a serious injury occurred. On this site, the employer did not ensure that provisions to ensure that emergency medical services were adequately available for an employee who sustained a serious head when the employee was struck with a portion of the pneumatic drill rig. The victim was placed and transported in a vehicle where they were transported while not under the care of emergency medical services.

(a) Jobsite - The Doosan DX 225LC Excavator had and unapproved drill rig attachment that was not covered by the load chart. (b) Jobsite - The Doosan DX 225LC Excavator had unapproved additional weights added to the counter weight.
