29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided: a. First Floor, Corridor in front of Ex G1 Procedure #1 Room, Crozier-Keystone Surgery Center at Haverford, 2010 West Chester Pike, Havertown, PA 19083 - The employer did not ensure that an employee, who was standing on a step ladder, while wiring an existing light into a junction box used the appropriate personal protective equipment, such as voltage-rated insulated gloves. There were live electrical parts operating at approximately 277 volts. This employee received an electric shock, fell onto a concrete floor, and sustained a fracture to the left arm femur near the elbow. This employee was exposed to both electric shock and fall hazards. Observed On or about March 31, 2015

First Floor, Corridor in front of Ex G1 Procedure #1 Room, Crozier-Keystone Surgery Center at Haverford, 2010 West Chester Pike, Havertown, PA 19083 - The employer did not ensure that an employee, who was standing on a step ladder, was protected from an energized circuit while wiring an existing light into a junction box. This employee received an electric shock, fell onto a concrete floor, and sustained a fracture to the left arm femur near the elbow. This employee was exposed to both electric shock and fall hazards.
