29 CFR 1926.252(a): An enclosed chute of wood, or equivalent material, was not used where materials were dropped more than 20 feet to points lying outside the exterior walls of the building:   (a) On or about April 20th, 2015, at a job site located at 5210 Blue Ridge Blvd. in Raytown, MO, employees were observed tossing roof debris over 20 feet into a trash trailer.  No chute was being used to control the debris and to protect workers from being struck by overhead falling objects.   Abatement certification is required for this Item.  Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.  NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    (a) On or about April 20th, 2015, at a job site located at 5210 Blue Ridge Blvd. in Raytown, MO, employees performing roofing work were observed standing directly at the edge of a flat roof over 20 feet above the ground.  No type of fall protection equipment or system was in place or in use at the time of the inspection.    Eder Parra (dba Eder Parra Roofing) was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 936614, citation number 1, item number 4, and was affirmed as a final order on December 2, 2013, with respect to a workplace located at 6140 Raytown Road in Raytown, MO.      Abatement certification is required for this Item.  Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
