An employee working for the company collapsed and died on June 9th 2015 at approximately 1500 hours while performing work for the company.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: The employer is failing to provide Hazard Communication training to employees either during their initial assignment or whenever a new physical or health hazard that the employee has not previously been trained for has been introduced into their work area. Some of the employees in the facility had not received training on the hazards of the chemicals/materials in the facility in which they could be potentially exposed.
