29 CFR 1910.22(a)(2): Where wet processes were used, drainage was not maintained, and false floors, platforms, mats, or other dry standing places were not provided when practicable:    (a)  In some extruder areas near the cold bath:  On April 20, 2015 and at times prior to; the employer exposed employees to slip hazards in that water from over flow or condensation of the cold bath kept the working surface wet.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one half hour:    (a)  In the maintenance shop:  On April 20, 2015 and at times prior to; the employer exposed employees to hazards associated with fires such as, but not limited to burns, smoke and fume inhalation or death, in that a flammable compressed gas cylinder was stored with two compressed gas oxygen cylinders.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a)  On the production floor in the Extruder area: On April 20, 2015 and at times prior to; the employer exposed employees to an in-running nip point hazard from the 4 pull rollers on each of the 16 old extruder machines.    (b)  On the production floor in the Extruder area: On April 20, 2015 and at times prior to; the employer exposed employees to an in-running nip point hazard from the single pull roller on each of the 15 new extruder machines.    Twitchell Technical Products, LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 314540691, citation number 1, item number 4 and was affirmed as a final order on October 13, 2010, with respect to a workplace located at 4031 Ross Clark Circle, Dothan, AL.

29 CFR 1910.215(b)(9): Guard for abrasive wheel machine where the operator stands in front of the machine was not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel:    (a)  In the maintenance shop:  On April 20, 2015 and at times prior to; the employer exposed employees to struck-by hazards from a pedestal grinder operating at 1800 rpm, in that the grinder was not equipped with an adjustable tongue guard.       Twitchell Technical Products, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.215(b)(9), which was contained in OSHA inspection number 314540691, citation number 1, item number 6 and was affirmed as a final order on October 13, 2010, with respect to a workplace located at 4031, Ross Clark Circle, Dothan, AL  36303.
