29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.   On the south side of the work site, the employer did not provide a scaffold ladder where the employees could access and egress the tubular welded frame scaffold exposing the employees to fall hazards of approximately 14 feet.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:   Employees engaged in masonry activities on a commercial building from a tubular welded frame scaffold without fall protection were exposed to fall hazards of approximately 14 feet to the lower level.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:   On the south side of the work site, the tubular welded frame scaffold poles did not have base plates for an adequate firm foundation exposing employees to fall hazards of approximately 14 feet to the concrete below.

29 CFR 1926.452(b)(2): Transverse bracing forming an "X" across the width of the scaffold was not installed at the scaffold ends and at least at every third set of posts horizontally (measured from only one end) and every fourth runner vertically. Bracing did not extend diagonally from the inner or outer posts or runners upward to the next outer or inner posts or runners:   On the south side of the work site, the employer did not ensure the tubular welded frame scaffold was erected with cross bracing at each tier exposing employees to struck by and fall hazards of approximately 14 feet to the concrete below.

29 CFR 1926.602(c)(1)(vi): Section 603(e) of American National Standard Institute, B56.1-1969 Safety Standards for Powered Industrial Trucks, as adopted by 29 CFR 1926.602(c)(1)(vi): When a truck was left unattended, load engaging means were not fully lowered, controls were not neutralized, power was not shut off, brakes were not set, and the key or connector plug was not removed:  On the south side of the work site, the employer allowed the forklift with the forks elevated to be unattended while installing masonry material on the commercial building exposing employees to fall and struck by hazards.
