29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    (a)All In One Restoration, LLC at 1592 North Perry Street, Denver, CO 80204:  On and before April 17th, 2015, the employer did not ensure that employees were using fall protection when working on a roof with an 8 on 12 pitch. The condition exposed employees to fall hazards of up to 13 feet.    Abatement note:  29 CFR 1926.502(d)(16)(ii) provides that personal fall arrest systems shall limit the arresting force on an employee to 1,800 pounds when used with a body harness (requires a shock absorbing capability).

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii):  (a)All In One Restoration, LLC at 1592 North Perry Street, Denver, CO 80204:  On and before April 17th, 2015, the employer did not ensure that employees had been trained by a competent person on fall protection requirements and equipment use.  The condition exposed employees to fall hazards of up to 13 feet.
